What would you do if one of your enemies came to you and asked for your help in saving one of their own?
Knockout finds himself thrown into this very situation when Smokescreen comes to him for help in saving Optimus's life. He is reluctant until he realizes that he has been given a prime opportunity to turn the war to the Decepticons favor. Smokescreen takes him to Optimus's side. Knockout realizes how easy it would be to make the kill, and how morally wrong it would be.

No doctor will go against the Hippocratic Oath willingly. Especially when it is a part of your Prime Directive in your core programing. All Transformers will not go against their Prime Directive, not even a Decepticon. (Except maybe Megatron, for all we know, his Prime Directive is to go against his Prime Directive.)
For Knockout, like Ratchet, their Prime Directive is to heal others. Knockout is loyal to Megatron, and his orders are to capture any survivors he may find in the ruins, and kill those who cannot walk themselves to the Citadel. It is a case of Prime Directive VS orders, normally a clear cut solution, except when Megatron threatens Knockout that if his orders are not followed, he will use Knockout as a scratching post. Knockout as we know, hates having his finish ruined. He is stuck between a rock and a hard place.
